is it better to go to faliure on each set or should i count reps and how many sets if they all go to faliure?

Try to hit failure every set, why would'nt you want to?
As for how many sets, do as many as you can handle within your workout, I drop my rest time to 30secs-1min on average now...

Go past failure, by utilizing shocking techniques like drop sets, supersets, rest-pause sets.
